DESCRIPTION:This event is aimed at bringing together local community based health organizations\, medical providers\, elected officials and other stakeholders to (1) highlight the social factors that influence the health and well-being of Brooklyn residents\, (2) showcase the value and importance of community-based health programs that address social determinants of health (3) strengthen partnerships between clinical and non-clinical care providers and (4) identify ways in which coordinated efforts can improve health and reduce disparate health outcomes for under-serviced Brooklynites.
