DESCRIPTION:The interest in population health is growing in New York\, as value-based payments gain ground and providers take on more risk. Many of these initiatives have taken place under the Delivery System Reform Incentive Payment Program (DSRIP)\, the state’s Medicaid reform effort. Once DSRIP expires in 2020\, will the momentum in population health continue? And how can successful programs be broadened to serve non-Medicaid populations? \nZip code is destiny for many health conditions: Poorer communities in New York have substantially higher rates of chronic disease and death from preventable conditions. A major focus of population health programs is to address the social determinants of health\, including education\, employment\, housing and food security. \nHealth systems\, payers and community groups face challenges in financing and scaling their initiatives and\, ultimately\, in measuring their success. Crain’s Health Care Summit will assemble thought leaders to engage in a frank\, thoughtful discussion of best practices in population health and the challenges ahead.
